Introduction of Product

Tadalafil is a selective reversible inhibitor of cGMP specific phosphodiesterase 5 (PDE5). When sexual stimulation leads to the local release of nitric oxide, PDE5 is inhibited by tadalafil, leading to an increase in cGMP levels in the cavernous penile, which results in the relaxation of smooth muscle, blood flow to the penile tissues, and erection without sexual stimulation.

The absorption rate and degree of tadalafil are not affected by food, so it may be taken with or without food. Duration of administration (morning or evening) had no clinically significant effect on absorptibility and degree, with a mean distribution volume of about 63 litres, indicating that tadalafil was distributed into tissues. At the therapeutic concentration, 94% of tadalafil was bound to the protein in plasma. Protein binding was not affected by renal function impairment. In healthy subjects, oral tadalafil had a mean clearance rate of 2.5L/hr and a mean half-life of 17.5 h. Tadalafil was excreted primarily as inactive metabolites, mainly from the stool (approximately 61% of the dose) and to a lesser extent from the urine (approximately 36% of the dose).

Treatment of pulmonary hypertension, male erectile dysfunction.

Tadalafil is mainly used to treat pulmonary hypertension in cardiovascular diseases. In addition, it can also be used to treat erectile dysfunction in men.

Taboo of Tadalfil

Clinical studies have shown that tadalafil can enhance the hypotensive effect of nitrate drugs. This is thought to be the result of nitrate drugs and tadalafil acting together on the nitric oxide/cGMP pathway. Therefore, patients who are taking nitrates in any form should not take this product.

Sex carries a potential heart risk for people with heart disease. Therefore, erectile dysfunction medications, including tadalafil, should not be used in patients with heart disease who are advised not to have sex. For patients who already have heart disease, doctors should consider the potential heart risks of sex. The clinical trials did not include patients with the following cardiovascular diseases, for which tadalafil is strictly prohibited:

● Patients who have had a myocardial infarction within the last 90 days

● Patients with unstable angina pectoris or who have experienced angina pectoris during intercourse

● Patients with heart failure who have met the New York Heart Association diagnostic criteria of grade 2 or above within the past 6 months

● Patients with refractory arrhythmia, hypotension (<90/50mmHg), or refractory hypertension

● Patients who have had a stroke within the last 6 months

● People with known allergy to tadalafil and its prescribed ingredients should not take this product.

Usage and dosage

1. Sildenafil for most patients, the recommended dose is 50mg, to be taken as needed approximately 1h before sexual activity and to be sexually stimulated after taking the drug. However, it can be taken at any time between 0.5 and 4h before sexual activity. Based on efficacy and tolerance, the daily dose can be adjusted between 25 and 100mg, and the drug should not be taken more than once a day. (1) The recommended starting dose for renal insufficiency is 25mg for patients with severe renal insufficiency (AUC and Cmax are nearly doubled when creatinine clearance is less than or equal to 30 mL /min). (2) The recommended starting dose for liver function impairment (e.g., in cirrhosis, AUC and Cmax increase by 84% and 47%, respectively) is 25mg.

2. Elderly Dose The starting dose for people over 65 years of age is recommended to be 25mg.

3. Dose for other diseases (1) The appropriate starting dose for those taking potent cytochrome P4503A4 inhibitor and erythromycin at the same time is 25mg. (2) It is recommended that sildenafil dose not exceed 25mg every 48 hours in patients taking HIV protease inhibitor prickle (such as ritonavir).

Side effects

Eyelid swelling, or eye pain as described, and conjunctival congestion are very rare side effects. Reports of adverse reactions caused by tadalafil were brief, mild, or moderate. These side effects mean that the data are only applicable to patients under age 75.
